FAQ

Physical Therapy & Sports Medicine For Additional Questions Please Contact Us At (281) 301-5090

Yes. The choice of where you receive therapy is always yours. While the majority of our patients come to us by physician referral/recommendation, many clients request to see us due to our strong clinical reputation in the community.

We operate in a direct access state where, if your insurance plan authorizes self-referral to physical therapy, a physician referral is not necessary for you to be seen by a licensed therapist. Please check with your insurance though as many health insurance plans require a physician’s referral for therapy to be covered. We also offer treatments, services or products that do NOT require a referral and are available on a self-pay basis.

We participate in the majority of major health insurance plans including Medicare and Worker’s Compensation. Please contact us prior to your first visit to see if we participate with your specific health plan.

As a service to our patients, we will gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.

Your therapist will conduct an evaluation of your condition lasting between 60-90 minutes. The evaluation is necessary to create a program designed specifically to address your needs for a maximum recovery.

– Referral slip from your medical doctor
– Your medical insurance card
– Your ID
– A copy of your medications list including dosages, if you have it
– If you are getting therapy as a result of a worker’s compensation claim or an automobile accident, you will also need to bring your claim information, including claim number.

Please arrive 15 minutes early on your first visit to fill out any necessary paperwork.

We recommend comfortable clothing such as sweatpants or shorts, a T-shirt or tank-top and sneakers.

Each visit will last 60 – 90 minutes on average. Times vary due to the treatment and exercises being performed.

In order for you to achieve the results you deserve, you will be given a treatment plan after your initial evaluation. It is important for your recovery that you attend every appointment outlined by your physical therapist and prescribed by your physician. You may start to feel better after only a few visits, but can relapse if you do not complete your entire course of therapy.

“I am so grateful to the physical therapists at Grand Oaks. Not only are they knowledgeable professionals, they are truly invested in their patients. Their compassion, understanding, and ability to treat the “whole” person, not just the injury, goes a long way in the healing process. I cannot thank them enough for helping me. I would, hands down, recommend them to anyone needing physical therapy.”

★★★★★

“Jacob and Beth are outstanding Physical Therapists! They know exactly what plan to implement for your recovery. I tend to ask a lot of questions because I like to know why they’re instructing me to do a particular exercise that may seem irrelevant to my recovery. I have to tell you, Jacob has gone above and beyond to answer any questions I may have, and even provides additional context to what he’s suggesting. I will come back to see Jacob, Beth, or any of their staff, because they’re all highly knowledgeable, caring and super friendly.”

★★★★★

“I’ve been to Grand Oaks and have had the best experience. Last year I was going for elbow pain. They did Needling (I think 4-5 sessions) and my elbow pain is now gone after suffering with it for over a year. More recently I saw Beth for PT after my double mastectomy. She’s helped me to rebuild my strength and get my range of motion back so quick! Definitely recommend this team.”

★★★★★

“Grand Oaks is not only convenient, it also has a wonderful team of PTs that make you feel at ease while you go through your paces! I will use them again!”

★★★★★

“10/10! I always look forward to going to PT! A few years ago, I injured my back doing gymnastics. Ever since, Grand Oaks has helped me regain my strength as well as my confidence. They are so easy to talk to I cannot say enough good things about all of the employees.”
